Behavioral Health

Our behavioral health program utilizes an evidence-based approach; assisting clients to remain independently at home. Nationally, hospitalization for persons with mental health disorders has increased faster than hospitalization for any other condition. Clients suffering from metal health disorders often lack adequate access to community mental health resources and have challenges with adherence to care plans. Access to community resource and assistance with adherence to care plans are crucial in preventing chronic relapse. Care teams ensure our clients have access to available service and promote adherence with the plan of care, while allowing the client to remain independently in the community.
