Diabetes Management

The diabetes care management program provides education, monitoring, and treatment of clients with Type I and Type II diabetes. The program includes skilled care by professional nurses and a multidisciplinary team under the orders of the client's physician to provide evidence-based treatment to promote positive health outcomes as well as an improved quality of life while allowing the client to remain in the comfort of their own home. This is achieved through patient and caregiver education, assessment, monitoring, client education materials, and collaboration with the client's physician with special attention given to the prevention of complications associated with diabetes.

Insulin and medication administration

Routine foot and nail care provided by a certified foot care nurse

Skin and foot care assessments provided by a skin care team under the supervision of a certified wound and foot care nurse
